In consideration of being allowed to participate in any way, and at any time in a sporting event sponsored by the Sunday Night Basketball (One Seed Mission), the undersigned: 

1. Acknowledges and fully understands that each participant will be engaging in activities that involve risk of serious injury, including permanent disability and death, and severe social and economic losses which might result not only from their own action, inactions or negligence, but the actions, inactions or negligence of others, the rules of play, or the condition of the premises or of any equipment used. Further, there may be other risks not known to us or not reasonably foreseeable at this time.

2. Agrees that prior to participating, they will inspect the facilities and equipment to be used, and if they believe anything is unsafe, will immediately advise the organizer, supervisor or referee of such condition(s) and refuse to participate.

3. Assume all the foregoing risks and accept personal responsibility for the damages following such injury, permanent disability or death.

4. Release, waive, discharge and covenant not to sue the One Seed Mission, and its related and affiliated organizations, their directors, commissioners, referees, employees, agents, facilities and sponsors from any and all liability to the undersigned, his or her heirs and next of kin for any and all claims, demands, losses or damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of One Seed Mission or its related “releases” enumerated above or otherwise.

5. NO DRUGS, VAPING (E-CIGARETTES), SMOKING, ALCOHOL, OR ANY OTHER ILLEGAL SUBSTANCES AND 100% SOBER ATTENDANCE IS REQUIRED. NO WANDERING AROUND IN SCHOOL FACILITY.

6. ALL GUESTS ARE INVITEE’S RESPONSIBILITY, only allowed with the organizer’s approval. (Not allowed until further notice) 

7. ALL PARTICIPANTS ARE TO FOLLOW THE CDC HEALTH GUIDELINES, such as but not limited to temperature check upon arrival, sanitization, COMPLETE VACCINATION to participate, etc. 

IF ANYONE BECOMES SICK (COVID RELATED SYMPTOMS) OR COVID POSITIVE, PLEASE INFORM THE LEAGUE ASAP. AT ANY TIME, THE RUN CAN SUSPEND INDEFINITELY WITHOUT REFUNDS.

8. All fees are non-refundable, absolutely no fighting, no swearing/cursing, all contentions are recreational purposes (copyright owned by the league), all participants MUST sign this waiver, and all terms subject to change. Any violations mentioned and unethical behaviors are all subject to ejection.

By checking the box below on this form, the participants have read the above waiver & release, understand that they are giving up substantial rights by signing it and sign it voluntarily.
